How to Use:
This is the latest ballast water reporting technology developed by NBIC. It is designed to transmit data accurately and reliably, while minimizing costly data transmission time. This form functions like a ballast water reporting computer program that resides on your PC. You can open it, add information, save that information, and print paper copies of the form, all without connecting to the internet.

When the Ballast Water Report is complete, please submit using one of the following methods:
Option 1: Submitt the form through email using the "send by email" button at the bottom of the PDF file. This buttom may not work with all shipboard email systems.
Option 2: If you are connected to the internet, you may use the "submit form 0nline" button at the bottom of the form. The form will connect to NBIC and upload the data you entered. A printable receipt will open in your browser providing proof of transmission.
Option 3: Use the "save as text file" button to extract the data from your report to an .xfdf file on your computer. This file can then be submitted as an attachment to an email to nbic@ballastreport.org.
Please note that if you are using a version of Adobe Reader that is 9.0 and newer you will not be able to save or submit your data electronically. The only option available with the newer versions of Adobe Reader is to print the completed form and fax it to (301) 261-4319. Since this is not the preferred method, please use an older version of Adobe Reader or select another method such as the web-format.
